Health history
Be prepared to share your relevant medical history, current medications, and any known conditions. This helps your physician make a safe, informed recommendation.
Your goals
Whether you’re focused on weight, recovery, performance, longevity, or hormonal health — articulating your goals helps your provider design the right protocol.
Lab work (if available)
Recent bloodwork is not required to begin, but if you have it, uploading it helps your physician personalize your protocol faster. Lab guidance will be provided as needed.
